Indications/Uses
Major depressive episodes, panic disorders w/ or w/o agoraphobia, social anxiety disorder (social phobia), generalized anxiety disorder & obsessive-compulsive disorder.
Dosage/Direction for Use
Major depressive episodes 10 mg once daily. Max: 20 mg daily. Panic disorder w/ or w/o agoraphobia Initially 5 mg for the 1st wk, subsequently increased to 10 mg daily. Max: 20 mg daily. Social anxiety disorder 10 mg once daily. May be decreased to 5 mg or increased to a max of 20 mg daily, if necessary. Generalized anxiety disorder & obsessive-compulsive disorder Initially 10 mg once daily. Max: 20 mg daily. Elderly (>65 yr) Initially 5 mg once daily. Max: 10 mg daily. Mild or moderate hepatic impairment & poor CYP2C19 metabolizers Initially 5 mg daily for the 1st 2 wk. Max: 10 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Patients w/ known QT-interval prolongation or congenital long QT syndrome. Concomitant use w/ medicinal products known to prolong QT-interval; non-selective, irreversible MAOIs; reversible MAO-A inhibitors (eg, moclobemide); reversible, non-selective MAOIs (eg, linezolid).
Special Precautions
QT interval prolongation. Do not start treatment in patients under linezolid therapy. Concomitant use w/ omeprazole or cimetidine. Liver problems. Sexual dysfunction. Abnormal bleeding/hemorrhage. Pregnancy & lactation. Elderly ≥65 yr.
Adverse Reactions
Abnormal dreams, fatigue, paraesthesia, pyrexia, restlessness, sinusitis, yawning.
Drug Interactions
QT interval prolongation w/ class IA & III antiarrhythmics, certain antipsychotics (eg, ziprasidone), TCAs, opioids (eg, methadone), certain antimicrobial agents (eg, moxifloxacin). Loop, thiazide, & related diuretics; laxatives & enemas; amphotericin B; high-dose corticosteroids.
